COLLINSVILLE QUILT SHOW ENTRY FORM

Welcome to the 2023 Collinsville Quilt Show. We are looking forward to an amazing exhibit of quilts!
The Show will take place on Saturday, April 29, 2023 from 10:00 AM to 4:00 PM at the
Veterans Community Center
903 W. Main Street
Fee to have your quilt on display is $10.
All patrons will be provided with disposable plastic gloves to protect displayed quilts.

A separate form is needed for each quilt entry

PAYMENT INFORMATION

Payment and entry is due no later than April 26, 2023. Please indicate how you will be paying to have your quilt displayed.

Liablility Statement

We will handle all quilt entries with great care and will supervise guests. The show cannot accept financial responsibility for show entries. By submitting this form you acknowledge your recognition and assumption of risk.
In consideration of the acceptance of the right to participate -Exhibitors of this entry form releases the City of Collinsville, American Legion Unit #2 Auxiliary, Collinsville Patchwork Quilters Club and anyone else connected with this event of and from all known or unknown damages, injuries, losses, judgements or claims from the cause whatsoever that may be suffered by an Exhibitor to his person or property. Each Exhibitor further agrees to indemnify all of the forgoing entities, firms, persons and bodies of and from any and all liability occasioned or resulting form the conduct of the exhibitors or any participant assisting or cooperating at the event.
